1. Services

Renaissance Medical provides telehealth consultations, medical weight loss programs, hormone replacement therapy, sexual wellness services, mental health support, skincare treatments, hair loss management, and quick sick visits. Our services are available to patients located in Florida and New York.

2. Eligibility

You must be at least 18 years of age to use our services. By using our services, you represent that you are legally capable of entering into binding agreements.

3. Patient Responsibilities

• Provide accurate and complete medical history and personal information.
• Follow prescribed treatment plans and medication instructions.
• Attend scheduled appointments or cancel/reschedule in accordance with our cancellation policy.
• Notify us of any adverse reactions or changes in your health condition.

4. Payment Terms

Our services are cash-pay and not covered by insurance. Payment is due at the time of service. We accept major credit/debit cards and other approved payment methods. Refunds are handled on a case-by-case basis.

5. Cancellation Policy

Patients may cancel or reschedule appointments up to 24 hours before the scheduled visit without penalty. Late cancellations may incur a $50 fee. No-shows may be charged a $75 fee.

6. Limitation of Liability

Renaissance Medical provides medical services to the best of our professional ability. We are not liable for outcomes that result from patient non-compliance, inaccurate information provided by patients, or circumstances beyond our control.

7. Intellectual Property

All content on our website, including text, graphics, logos, and images, is the property of Renaissance Medical, LLC and is protected by applicable intellectual property laws.

8. Modifications

We reserve the right to update these Terms of Service at any time. Continued use of our services after changes constitutes acceptance of the updated terms.
